CHECKPOINT 1: WHEN WILL I GET A COVID-19 VACCINE?
The Federal Government Department of Health determines the eligibility criteria for Covid-19 vaccinations. Please check the following to check the eligibility criteria.

CHECKPOINT 3: YOU MUST HAVE A VALID MEDICARE CARD NUMBER
Medicare card holders can book online. Prior to booking your vaccine, make sure your Medicare card is under your name, is valid and active. If you have a reciprocal Medicare card number, your card must be valid and active.
Non-Medicare card holders can book your Covid-19 Vaccine by checking the Eligibility Checker and finding the nearest clinic. Unfortunately, we cannot take vaccine bookings for non-Medicare card holders.
CHECKPOINT 4: BOOK ONLINE / COMPLETION OF MANDATORY FORMS
Covid-19 vaccinations can only be booked online via HotDoc. Book via the app or our BOOK ONLINE button on our website. We do not make bookings over the phone.
Once you make an online booking, IMPORTANT INFORMATION will be sent to you to complete PRIOR to your vaccine appointment.
If we do not receive the completed forms prior to your vaccination appointment, we will cancel the booking.
Existing Patients
- You are required to complete the Covid-19 Vaccine Consent Form prior to your vaccine booking.
Please ensure you take the time to read and complete prior to your vaccine booking.
New Patients
- You are required to complete our New Patient Registration Form prior to your vaccine booking.
- You are also required to complete the Covid-19 Vaccine Consent Form prior to your vaccine booking. Please ensure you take the time to read and complete prior to your vaccine booking.
3RD DOSE VACCINE: IMMUNOCOMPROMISED INDIVIDUALS
